SUMMARY:Human Factors in Accident Investigation
DESCRIPTION:South Wales Regional Group Meeting.    Case studies from various industries will be presented by Simon Monnington and Eryl Marsh.    Simon Monnington is a Subject Matter Expert\, Investigations for BP Investigation and Learning Dept (Refining). After many years experience working first with the human sciences team at the Health & Safety Laboratories in Buxton\, and subsequently as a Specialist Inspector of Human Factors for the Health & Safety Executive in Cardiff\, Simon has more recently joined BP. Simon has worked in various industry sectors ranging from Fairgrounds to Oil & Gas\, and most things in between!    Eryl Marsh is an HM Specialist Inspector of Health and Safety (Human Factors). Eryl has worked for the Health & Safety Executive for a number of years\, initially as a Regulatory Inspector and subsequently as a Specialist Inspector of Human Factors. Her main industry sectors of focus are the onshore chemicals\, oil and gas industry.    All are welome but you must register your interest by 5th November.
